Regence Blue Cross/Blue Shield -
$1500 deductible. After Deductible, Employee pays 20% for Office Visits & Prescriptions. Monthly Premiums are contributed towards a Health Savings Account. Great opportunity for those who don’t use medical on a regular basis.
Each insured person will have a $25.00 calendar year deductible. The calendar year maximum is $1000.00 for each member covered. Preventive services are paid at 100% of usual and customary charges.
This plan is designed to allow insured members to use all Willamette Dental facilities in Oregon. The co-payment per visit is $15. There is no annual maximum paid benefit. Orthodontic benefits are provided on this dental plan.
Vision Service Plan (VSP)
VSP plan description is available in the Human Resources Office. VSP member doctors can be determined online at www.vsp.com or by calling 1-800-877-7195. Exams $10, if using a member doctor Lenses/Frames $25 materials co-payment (Adults: every 24 months, Children: every 12 months)
Regence Life & Health
Each employee who qualifies for insurance benefits, is entitled up to $5,000.00 dollars of life coverage as an employee benefit. Qualified employees may also purchase supplemental coverage for themselves, their spouse or children at an additional cost.
Colonial accident, disability, cancer, and intensive care plans are also available through payroll deduction.
403(b) Retirement Plans:
Voluntary 403 (b) Plan
Employees may elect to use pre-tax money and invest in the PRS voluntary 403(b) retirement plan. The Summary Plan Description is available in Human Resources.
Matching 403 (b) Plan
Employees who are: over 21 years of age, have worked at least 1,000 hours, and have been employed with PRS for at least 1 calendar year are eligible for up to a 4% match. PRS will match up to 4% of your pre-tax contribution. The Summary Plan Description is available in Human Resources.
Roth 403 (b) Plan
Employees may elect to use post-tax money and invest in the PRS voluntary 403(b) retirement plan. The Summary Plan Description is available in Human Resources.
The Flexible Spending Account:
Is a flexible spending account program that allows employees to divert money on a pretax basis into an account to help pay for medical and dental expenses, dependent care expenses, and employee insurance premiums that employees would normally have to purchase with aftertax dollars.
Paid Vacation and Holidays:
Employees who are working full-time or part-time and have completed 12 months of active employment will be eligible for annual vacation time with pay on the anniversary date of their latest date of hire at the following rates:
- Two weeks (10 days) paid vacation for employees first through fourth anniversary dates of continuous service.
- Three weeks (15 days) paid vacation for employees fifth through ninth anniversary dates of continuous service.
- Four weeks (20 days) paid vacation for employees tenth and greater anniversary dates of continuous service.
Vacation is calculated according to the employee's total paid hours in the preceding year.
Scholarships and Loan Program:
Scholarships are available to employees for a portion of tuition and fees up to $500 for each quarter and for a period not to exceed four years. Scholarships are administered through the RVM Foundation and amounts will be based on the availability of funds. Eligible employees must have worked for the company for at least one year and a minimum of 1,000 hours. All scholarships are subject to supervisor recommendation based on motivation and job performance.
Golf Course Discounts:
A 20% discount is available to all employees on purchases made at the Quail Point Golf Course.
Fitness Facilities and Pool:
All employees are welcome to use the Resident Pool and Fitness Center on a limited schedule. Schedules are available in the Human Resources Department.
